Health Consultation Agreement
The town of Hermon provides the Day Care with 911 Emergency Service. We have explicit written directions to the day care next to the phone. All of the employees are made aware of the content and location of this document.
Immunization Record
The Fun House Day Care requires that an Immunization Record form be completed and on file for any child that is not attending public or private school.
